How To Obtain A Work Permit

1. Download and print out both pages of the Work Permit Application (coming soon). You will need Adobe Reader, which can be downloaded for free at Adobe.

2. Forms are also available from Mrs. Wright in the Career Center.

3. STUDENT to complete Section 1.

4. EMPLOYER to complete and sign Section 2.

5. PARENT/GUARDIAN to complete and sign Section 3.

6. EMPLOYER also to sign and date PAGE 2.

7. STUDENT brings both pages to the Career Center between 8 a.m. to 4 p.m.

8. Mrs. Wright & your Facilitator will complete Section 4 and issue the work permit.

Work permits will be processed when students bring completed application to the Career Center.
